Non-invasive Caries Management

Non-invasive caries management reverses or arrests tooth decay without drilling or fillings, focusing on remineralization and plaque control. Key methods include high-fluoride toothpaste (5,000 ppm), 5% sodium fluoride varnish, dental sealants, silver diamine fluoride (SDF), and antimicrobial agents like arginine. This approach is most effective for early, non-cavitated lesions. Curodont is another non invasive caries management. 

Silver Diamine Fluoride (SDF) is a non-invasive, antimicrobial liquid (typically 38% FDA-approved) used to stop active dental caries without drilling or anesthesia. It works by killing bacteria (silver) and remineralizing tooth structure (fluoride). Ideal for children and high-risk patients, it arrests decay but causes permanent black staining on treated lesions. 
Key Aspects of SDF Caries Management:
  • Mechanism: Silver ions kill bacteria and prevent biofilm, while fluoride strengthens enamel and dentin.
  • Application: The procedure is fast, painless, and does not require local anesthesia or sedation. It involves cleaning the tooth, drying it, and applying the solution for 1–2 minutes.
  • Effectiveness: Highly effective at arresting decay, often used to postpone more intensive restorative work.
  • Limitations: Treated areas will stain black permanently. It is not a replacement for, but rather an alternative to, traditional fillings or crowns.
  • Target Patients: Ideal for young children, patients with special needs, or those with dental anxiety.

  • Frequency: Often applied every 3 months to ensure the caries remain inactive.
  • Procedure: Involves isolating the tooth, removing debris, and applying the solution, followed by a waiting period of at least 60 seconds.
  • Safety: Studies, such as those conducted in Hong Kong, have shown no significant adverse effects in large-scale applications. 
Curodont Repair is an FDA-approved, non-invasive, peptide-based treatment designed to reverse early-stage (non-cavitated) enamel caries without drilling or fillings. Clinical reviews indicate it is highly effective, with some reports showing over a 90% success rate in stopping or reversing early decay and a significant increase in caries arrest compared to standard treatments.

  • Effectiveness: Studies show a large increase in caries arrest (relative risk 1.82) and a 32% average reduction in lesion size.
  • Mechanism: It acts as a guided enamel remineralization agent, helping to rebuild tooth structure.
  • Application: The procedure is quick (under 10 minutes), painless, and suitable for all ages, making it ideal for treating white spots or initial demineralization.
  • Benefits: It acts as a "non-drill" solution, preserving natural tooth structure.
  • Limitations: While promising, some clinical trials have high risks of bias, and longer-term studies are recommended.
